Hi there,
 
i ve arrange one charity golf tournament in Bukit unggul Country Club bangi, really need you guys to support me. there will be media tv and newspapers to do the coverage for this program as datuk seri azalina and few more vvip will be there. pls support.

I think what you can do is to scan the forms in PDF format (you need Adobe to do this), save it in your PC and upload it here..

ok thanks!! but wat i meant was Dr. Maria ni representing which organisation? & the charity ni utk badan amal yg mana? sori kalau terlebih byk tanya sbb nak promo kat member2 lain about this event  ;)
